Childcare Forms: Authorizations

I've been busy busy gearing up for my next adventure! I will be caring for some darling little kiddos in my home beginning next school year. While I will not be required by my state to be licensed, I did create a handbook (more on that later) and forms. This way both parties know what to expect (with no suprises later!).


Below you will find 2 Authorization forms- these can be opened in Scribd then downloaded in Word. You can even edit these documents!
